Would you like to transform the health and wellbeing of people in local communities?

The Wellbeing Practitioner post holder will have knowledge/experience of local community health needs and will work with individuals and groups (aged 16+) in homes and community settings over a period of eight to twelve weeks to facilitate behaviour change particularly in the areas of healthy eating, mental wellbeing, physical activity, stopping smoking and reducing alcohol intake, improving mental wellbeing and reducing social isolation.

The Wellbeing Practitioner will work closely with a Senior Wellbeing Practitioner in a range of settings in the communities across County Durham to generate clients for the service, carrying out information gathering and promotion in the local areas, signposting clients to relevant services and enabling them to take up services within the community. The post holder will liaise with a variety of partners, community and voluntary organisations across the hub area and the wider service.

The post holder will act as an integrated member of a team, demonstrating a flexible approach to working across the hub area and County Durham to ensure the needs of the local community are met.Excellent communication skills and local knowledge are essential in order to engage and work with members of the public. Willingness to undertake mandatory and role specific training within a specified timescale will also be essential.

Salary: £16,968 to £19,852 pa pro rata

Area bases: Newton Aycliffe, Willington, Easington and Stanley

Hours: up to 37.5 hours per week
